Cigna—We bill these cases Out-of-Network at $150 per case. Patients will typically owe around $50 per case after carrier pays; any patient's case debt may be relieved by calling Dr. Howard if patient does not wish to pay. Uninsured patients are billed $50 per case. This also may be relieved if needed by calling Dr. Howard. Gratis/professional courtesy cases are available for client employees and indigent patients.
