Applications Pending Approval
Out of Network Cases (we will bill these cases at $150.00 per H&E case, patients will typically owe around $50.00 per case after carrier pays; ANY patient’s case debt may be relieved by calling Dr. Howard if patient does not wish to pay). Uninsured patients are billed $50.00 total per case, also may be relieved by calling Dr. Howard if needed.
Gratis/professional courtesy cases are also available for client office employees and indigent patients, simply call Dr. Howard.
